veterinary consent before I can treat your horse.
Please print this form and get your veterinary surgeon to sign it prior to my
visit. Alternatively, I can contact your vet and will ask you to sign a form
stating that you are happy that I have gained verbal veterinary permission
before starting treatment on your horse.
